Summary of Therapeutic Elements Center for Massage Therapy Loyalty Rewards Program Terms and Conditions:

Eligibility:

– Must be a client/guest of the Therapeutic Elements Center for Massage Therapy Loyalty Rewards Program in the US, and be 13 or older.

– Reward points statements are sent out monthly to guest w/ marketing messages enabled.

Earning Points:

– 1000 points for referring another client

– 500 points for purchasing VIP Membership

– 50 points for booking appointments online

– 50 points for rebooking next appointment for any visit

– 50 points for client’s first visit

– 1 point equal to any service purchase price

– 1 point equal to any retail purchase price

– 1 point equal to any gift card purchase price

– 1 point equal to any package purchase price

Redeeming Points:

– 1 point = $0.01 (Example – 500 points = $5)

– Points can be redeemed upon checking out

– Minimum $5 to redeem

– Points can only be redeemed at Therapeutic Elements Center for Massage Therapy

Refunds & Returns:

– Points will be rescinded if a purchase is returned.

Reward Conditions:

– Rewards may be taxable.

– Members are responsible for sales tax, gratuity, and reporting taxable items on their tax returns.

– Reward points are not awarded on gratuity.

– Reward points are non-transferable.

– Reward points expire after 12 months of inactivity.

Termination:

– Therapeutic Elements Center for Massage Therapy may cancel, change, suspend, or modify any aspect of the program or rewards at any time.

– Therapeutic Elements Center for Massage Therapy may terminate or suspend a member’s participation for breach of terms and conditions or for actions inconsistent with the program’s intent.

Governing Law:

– Governed by the laws of the State of Florida.

– Members consent to jurisdiction and venue in Florida courts for matters arising in connection with the program.
